Every player participating in an AFA tournament, league, or event must have a current waiver on file.

One AFA Waiver System Use this page for tournaments, leagues, and future AFA events. Players must complete their own waiver. No completed waiver = no participation.

Before You Begin

Founders Cup Players Select “Founders Cup 2026” and enter your team name and registration code exactly as provided by your captain.

AFA Participant Waiver & Acknowledgements

Voluntary Participation & Assumption of Risk

I understand that participation in futsal and related athletic activities involves inherent risks, including falls, collisions, contact with other participants, playing-surface hazards, equipment-related injuries, serious bodily injury, illness, property damage, or death. I voluntarily choose to participate and knowingly accept these inherent risks.

Release of Liability

To the fullest extent permitted by law, I release and hold harmless Arizona Futsal Association (AFA), its owners, organizers, employees, volunteers, officials, sponsors, partners, and event facilities from claims arising from the ordinary risks of my voluntary participation, except where such a release is prohibited by applicable law.

Medical Authorization

If I become injured or ill and cannot provide consent, I authorize event staff to contact emergency services and seek reasonable emergency medical assistance on my behalf. I understand that I am responsible for costs associated with my medical care.

Rules & Conduct

I agree to follow AFA rules, referee decisions, facility policies, and standards of sportsmanship. Unsafe, abusive, threatening, or seriously unsportsmanlike conduct may result in removal from a match or event without refund.

Photo & Video Release

I grant AFA permission to photograph, record, and use my likeness in event photography, video, social media, website content, advertising, and promotional materials related to AFA, without additional compensation.
